What is the SFC Program?

The SFC program with Health Force of Georgia offers financial and support services to Georgia families that deliver care at home for a senior or disabled person. If your family member is elderly or has a disability and needs help with daily tasks, like getting dressed or eating, the SFC program can assist. 

Who is Eligible?

To be eligible for the SFC program, you must:

  • Be a family member of the person you are caring for. Family members include spouses, parents, children, siblings, grandparents, and grandchildren.
  • Be able to provide at least 16 hours of care per week.
  • Not be employed outside the home.
  • Not have any criminal convictions preventing you from working with children or vulnerable adults.

The Caregiving Services

The caregiving services that can be provided under the SFC program include:

  • Assistance with activities of daily living (ADLs), such as bathing, dressing, and toileting
  • Assistance with instrumental activities of daily living (IADLs), such as cooking, cleaning, and managing finances
  • Supervision and companionship
  • Respite care
